Medi-Cal is the largest Medicaid program in the United States, covering more than 15 million Californians — over a third of the state's population. That includes children, working adults, seniors, people with disabilities, and individuals experiencing homelessness or housing insecurity.
A program at that scale only works if there is a serious effort to coordinate care for the members with the most complex needs. Without programs like ECM, those members fall through the cracks of a fragmented system — repeating tests, missing follow-ups, and ending up in the ER for conditions that could have been managed in the community.
Our work is built on top of that public infrastructure. Members do not pay anything out of pocket for ECM; the benefit is fully covered through their Medi-Cal managed care plan.
