For members
Self-Referral
If you're a Medi-Cal member ready to start services, you can refer yourself. We'll respond within one business day and walk you through next steps.
For our team
Internal Referral
Pacific Health staff and care teams: route a member from your panel into the right service line — ECM, CHW, behavioral health, housing, or nutrition.
For partner organizations
External Referral
Clinics, hospitals, county agencies, and CBOs: refer a Medi-Cal member to Pacific Health. We respond within one business day and keep you looped in on enrollment.
