Pacific Health Group is required by law to maintain the privacy of your protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms currently in effect.

We use and disclose PHI for treatment, payment, and healthcare operations—including coordinating care across providers, processing claims with your Medi-Cal health plan, and improving the quality of our services.

We will not sell your PHI or use it for marketing purposes without your written authorization.
